This template may be used on Wiktionary entry pages to quote Richard Hooker's work The Answere of Mr. Richard Hooker to a Supplication Preferred by Mr Walter Travers to the H.H. Lords of the Privie Counsell (1st edition, 1612). It can be used to create a link to an online version of the work at Google Books (archived at the Internet Archive).
The template takes the following parameters:
