This template may be used on Wiktionary entry pages to quote from an English translation by George Herbert of Luigi Cornaro's work A Treatise of Temperance and Sobrietie (written before 1634), which was published together with Leonardus Lessius's work Hygiasticon: Or, The Right Course of Preserving Life and Health unto Extream Old Age (1st edition, 1634). It can be used to create a link to an online version of the work at the Internet Archive.
The template takes the following parameters:
